Botox vs fillers: what is the difference?

A simple guide by Dr Adriana M. Ozery, Doctor Picasso, Limassol

Botox and dermal fillers are the two most popular injectable treatments, and they are often mentioned together — but they work in completely different ways. Knowing the difference helps you understand which one (or which combination) suits your face.

How Botox works

Botox is an anti-wrinkle injection that temporarily relaxes the specific muscles that create expression lines. When the muscle moves less, the skin above it creases less, so existing lines soften and new ones are less likely to form.

It is best for dynamic lines — the ones that appear when you move your face: frown lines (the “11 lines”), forehead lines and crow’s feet. It is also used medically for teeth grinding and jaw slimming (masseter), migraines and excessive sweating.

How dermal fillers work

Dermal fillers are made from hyaluronic acid, a substance that occurs naturally in the skin. Instead of relaxing muscles, they add volume and support exactly where it is needed.

They are best for volume and shape: fuller or more defined lips, cheeks, chin and jawline, under-eye hollows (tear trough) and deeper folds such as marionette lines.

The key differences at a glance

What they do: Botox relaxes muscles; fillers restore volume and contour.

Best for: Botox for lines caused by movement; fillers for lost volume, shape and lines that are visible even at rest.

When you see results: Botox gradually takes effect over about 1–2 weeks; filler results are visible straight away and settle over around 2 weeks.

How long they last: Botox usually lasts around 3–4 months; fillers typically last around 6–12 months, depending on the area and the product.

Can you have both?

Yes — and they often work best together. For example, Botox can soften frown lines while a filler restores lost volume in the cheeks, giving a more balanced, refreshed result than either treatment alone.
